The figures on this site are published benchmarks, not Merid results. The hour a denial costs, 45–60 minutes, is an estimate built from CAQH Index task times and BLS wage data. The $17 cost to contest a commercial denial and the 40–66% of denied claims never resubmitted (commercial to Medicaid) are Dunn et al., QJE 139(1) 2024, corroborated by Hoagland et al., JAMA Network Open 2024. The 43–67% range is the share of prior‑authorization denials overturned on appeal in KFF’s 2026 analysis of insurer transparency filings, from ACA marketplace plans to Medicare Advantage.
